Gilead has reported 12wk. placebo-controlled induction data from the dose-ranging P-II (SWIFT) study evaluating once-daily oral emvistegrast (GS-1427; 200mg, 75mg or 25mg) vs PBO in adults (aged 18–75yrs.) with moderately to severely active UC, presented as a late-breaking abstract at UEGW 2026
At Wk. 12, emvistegrast 200mg met the 1EP of clinical response (66% vs 37% for PBO), along with clinical remission (31% vs 8%), HEMI (25% vs 10%), mucosal healing (24% vs 10%) & endoscopic improvement (39% vs 12%); AEs occurred in 36% (all emvistegrast doses) vs 37% (PBO) of pts & were largely Grade ≤2, with SAEs in 2% of emvistegrast pts & none on PBO (all deemed unrelated to study drug)
78% of pts were naive to advanced therapies or had not failed any; SWIFT remains ongoing with a maintenance phase (through Wk. 52) & an extension (through Wk. 116), and Gilead plans to initiate a P-III monotherapy study in 2027; emvistegrast is an investigational, internally discovered oral α4β7 integrin inhibitor
